Primary signet-ring cell carcinoma (SRCC) is one of extremely rare colorectal diseases in children. It has frequently metastasized to other sites of the body and presented with worse prognosis. We report a case of primary SRCC of the descending colon in a 14-year-old girl who presented with lower abdominal pain and vomiting. The diagnosis of SRCC with peritoneal seeding was made with abdominal CT scan and colonoscopic biopsy. We performed the left hemicolectomy and administered the intense chemotherapy regimens for SRCC, and then the patient¡¯s disease status had been stable for 14 months. However, her clinical course had deteriorated in spite of the multimodal chemotherapy regimens. She died of multiple metastasis 24 months after the initial diagnosis of SRCC. Since the prognosis of primary SRCC of the colon is extremely poor, early diagnosis and aggressive treatment strategies are needed.
